New Patient Forms
Dr. Jackson and his team go above and beyond to make your first visit to Jackson Dental as smooth and painless as possible. Before your first visit, our team members will make sure to obtain important background information about your medical and dental history. This 80-minute session will include:
- Full mouth series of x-rays: A set of bitewing, or "cavity detection," x-rays will be taken that allow the doctors to see in between your back
teeth. This is one of the most common spots for cavities to develop. A
set of these x-rays will be taken every year, and they're comprised
of a quick 2 series x-ray on each side. The remaining x-rays will be
periapical xrays (PA's), which will show any abnormalities with the root or bone.
- Complete full-mouth charting of your gums:
A vital component of checking for and monitoring gum disease and
overall oral health is measuring "attachment loss." We want to be sure
that your teeth have the appropriate bone and tissue support to allow
for proper oral and systemic health. The team will take measurements for
each tooth while an assistant records them in your file.
- Comprehensive oral examination: Our doctors will analyze the information we've gathered and perform an oral exam to get the most accurate picture of your health. The doctor will then discuss his findings with you and go over what, if any, further treatment is suggested before your next cleaning. He will also address and questions or concerns you may have.
